People Incorporated serves people with mental illness in the Minneapolis and Saint Paul metro area. In 2017, we reached over 12,000 individuals through more than 60 different programs. Helping people with mental illness requires great flexibility and creativity. That's why we start with the person first...developing innovative, effective services that provide the care, resources, and connections that will help each person in the best way possible. Join us in our mission to support mental health and wellness in our community through collaboration and integration of care!

We are honored to be selected as one of the clinics to launch the first Certified Community Behavioral Health Clinics (CCBHC) within the United States!  CCBHC are community clinics that offer comprehensive, coordinated and integrated care to children and adults with complex mental and chemical health conditions.  CCBHC will provide outreach, increase access, improve services, and serve as a "one-stop-shop" to those who are currently underserved with mental health and substance use disorders.

We are searching for a Care Coordinator to be part of the CCBHC program! Care Coordinators are responsible for ensuring that our consumers are connected with the services that they need and for collaborating with staff across the agency and with external agency partners.  Responsibilities will include:

  • Assessing consumer needs in the Preliminary Screening process
  • Connecting with services, and ensuring continuity of care among internal providers and between internal and external providers
  • Facilitating internal and external collaboration to develop person-centered or family-centered integrated treatment plans
  • Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services
  • Documenting and maintaining accurate and timely case notes
  • Providing assistance to consumers in making appointments and obtaining supports necessary to keep appointments
  • Ensuring that a crisis plan has been developed and implemented at your designated programs
  • Facilitating communication between providers within a schedule that meets outlined timeframes
  • Developing close working relationships with internal and external referral resources

Required Qualifications:

  • Valid driver's license, excellent driving record and current auto insurance

  • Bachelor's Degree in Human Services and 2,000 hours (1 year) of supervised experience working with people with severe and persistent mental illness
  • In lieu of degree, a high school diploma or equivalent and proof of 10,000 hours (5 years) of supervised experience working with people with severe and persistent mental illness may be substituted.
