Our Plus Program is seeking a full-time Case Coordinator. This position will be responsible for the day-to-day oversight and management of assisting enrolled Plus students to achieve success in reaching personal, educational and career goals. The Case Coordinator will consult with internal and external teams and parents/guardians to assure student success in the program. The Case Coordinator will:

  • Develop and maintain respectful relationships with adolescent clients, parents and colleagues
  • Work with students and teams to support student participation and attendance and resolve any behavioral concerns
  • Provide support and stabilization to the students during crisis situations
  • Assist in the creation of individual development/goal plans
  • Identify short and long term goals with school staff and students
  • Introduce and refer students for other physical/mental/chemical health needs

Required Qualifications:

  • Associate's Degree in Human Services or related field
  • Current and valid driver's license and auto insurance
  • Five years of experience working with adolescents and families

Preferred Qualifications:

  • Bachelor's Degree in Human Services or related field
  • Two years of experience in working with adolescents and families
  • Knowledge of education and county probation and human services system
  • Knowledge or experience with school systems
