The Behavioral Health Home program utilizes a multi-disciplinary team to deliver person-centered services designed to support a person in coordinating care and services while reaching his or her health and wellness goals.  We engage our clients and their families in moving toward stability, recovery and self-sufficiency.

We are searching for a Systems Navigator to act as the central point of contact for clients and ensure that they get their behavioral and physical health needs met in a coordinated way.  Responsibilities will include:

  • Coordinating care between behavioral and physical health specialists
  • Educating clients regarding possible resources and supporting clients in accessing those services
  • Supporting clients thought ongoing motivational
  • Assessing client needs to support the creation of individual development plans
  • Collaborating with a multi-disciplinary mental health team
  • Promoting recovery, stability and self-sufficiency

Required Qualifications:

  • High school diploma and minimum age of 21 years
  • Excellent driving record, reliable car, and current auto insurance


  • Bachelor's degree in Human Services
  • One year of full-time work experience with people with serious mental illness


  • Three years of full-time work experience with people with serious mental illness
