We have an open position for a full-time Mental Health Practitioner II. Our Assertive Community Treatment (ACT) program assists long-term homeless adults who have a serious and persistent mental illness. Our clients are referred to the program from staff at shelters by People Incorporated's street outreach team, other outreach workers in Hennepin County, and community agencies.

Each Mental Health Practitioner II is one of eight team members serving a primary caseload of approximately 8-10 clients. All team members are responsible to serve and support the ACT Homeless team caseload of 40.  Responsibilities include:

  • Establish integrated relationships with client families and community providers
  • Complete Functional Assessments and Individual Treatment Plans for clients
  • Provide case management, facilitate groups
  • Complete on-going Functional Assessments, LOCUS, and Individual Treatment Plans for each client
  • Provide case management, facilitate groups, and teach rehabilitative skills
  • Monitor psychiatric symptoms and respond immediately in crisis situations

Required qualifications:

  • Master's degree in social services


  • Bachelor's degree in social services
  • 4,000 hours of supervised experience in the delivery of services to people with serious and persistent mental illness

Preferred qualifications:

  • 6,000 hours of Case Management experience
  • Experience working with adults diagnosed with serious and persistent mental illness
  • Field/mobile unit experience
