We are honored to be selected as one of the clinics to launch the first Certified Community Behavioral Health Clinics (CCBHC) within the United States!  CCBHC are community clinics that offer comprehensive, coordinated and integrated care to children and adults with complex mental and chemical health conditions.  CCBHC will provide outreach, increase access, improve services, and serve as a "one-stop-shop" to those who are currently underserved with mental health and substance use disorders.

We are searching for a Care Coordinator to be part of the CCBHC program! Care Coordinators are responsible for ensuring that our consumers are connected with the services that they need and for collaborating with staff across the agency and with external agency partners.  Responsibilities will include:

  • Assessing consumer needs in the Preliminary Screening process
  • Connecting with services, and ensuring continuity of care among internal providers and between internal and external providers
  • Facilitating internal and external collaboration to develop person-centered or family-centered integrated treatment plans 
  • Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services
  • Documenting and maintaining accurate and timely case notes 
  • Providing assistance to consumers in making appointments and obtaining supports necessary to keep appointments
  • Ensuring that a crisis plan has been developed and implemented at your designated programs
  • Facilitating communication between providers within a schedule that meets outlined timeframes
  • Developing close working relationships with internal and external referral resources

Required Qualifications:

  • Valid driver's license, excellent driving record and current auto insurance

AND EITHER

  • Bachelor's Degree in Human Services and 2,000 hours (1 year) of supervised experience working with people with severe and persistent mental illness
  • In lieu of degree, a high school diploma or equivalent and proof of 10,000 hours (5 years) of supervised experience working with people with severe and persistent mental illness may be substituted. 

OR

  • Master's Degree in Human Services

Preferred Qualifications:

  • Master's degree in human services
  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings
  • Knowledge of Harm Reduction, Motivational Interviewing and Person-Centered approaches
